Register With The Practice?
To register with the practice new patients no longer need to provide proof of identity and all they need to provide is recent (within the last 3 months) proof of address (utility bill, tenancy agreement, council tax bill, bank statment and suchlike). Before registration can be completed or any clinical appointment
booked, new patients must attend an appointment for a new patient medical check with the healthcare assistant.
Applicants are invited to download and print off the GMS1 form and complete all relevant boxes and bring it together with the relevant credentials as shown above.

How Do I....
Notify Changes To My Personal Details?
It is vital that the records we hold are accurate and up to date. Any changes to patient's address or name/D.O.B can only be done with evidence i.e. proof of address documents, marriage/divorce certificate, passport and such like.
